• No appointment needed — first come, first served while supplies last.

TIER 1 — MEET WITH US

• Sign into the kiosk for help. Pick a goal: Food/Clothing/Shelter, Body/Health/Recovery, or I Don't Know.

• Talk to a Care Coach for a short 15-minute conversation.

• We verify your Medi-Cal and unlock free services you may not know about.

• YOU NOW GET TO PICK 1 REWARD

TIER 2 — WORK WITH US

• Tell us about a health condition like addiction, anxiety, or a long-term sickness.

• Share your living situation and set goals with your care coach.

• Agree to meet regularly by phone, video, or in person.

• YOU NOW GET TO PICK 2 REWARDS

TIER 3 — NEED BIG HELP

• Worked with our Care Coach and made all appointments.

• You need a lot of help or many kinds of help and want a care team.

• You need funds to stay housed or require health help.

• We introduce you to a special care team that connects you to bigger programs.
