Transportation

In rural and semi-rural parts of the Central Valley, a medical appointment can require two bus transfers and four hours of travel — each way. For people who are unhoused, caring for children, or working hourly jobs, that's simply not possible.

These programs exist. They're funded. And most people who qualify never access them — not because they don't deserve them, but because nobody told them they existed.

What Accessible Health Support Can Actually Look Like

Who May Qualify for Free Health and Wraparound Support

You may qualify for additional free services through Medi-Cal if you are:

  • Unhoused or in unstable housing — sleeping in your car, a shelter, or couch-surfing

  • Going to the ER often — two or more visits a year, especially for things a regular doctor could handle

  • Living with serious mental illness — depression, bipolar disorder, PTSD, schizophrenia, anxiety disorders

  • In recovery or struggling with substance use — including alcohol, opioids, meth, or other substances

  • Managing two or more chronic health conditions — diabetes, heart disease, asthma, COPD

  • Recently released from jail or prison — especially in the past 12 months

  • Pregnant or postpartum — particularly if you're also dealing with housing or mental health challenges

  • Living with a disability — physical, developmental, or intellectual

  • A frequent hospital patient — especially if you've had recent hospitalizations
