Health Helper Second Chance Session

This 15 minutes session is just for you. It's for people whose Medi-Cal or Medicare changed — maybe it lapsed, you need to renew, you moved, you switched plans, or you only had the blue and white or poppy card and now you have a health plan. This is your last chance to keep getting help from Help Is Hope. We have a lot of people who need our help, and not enough staff to track down everyone who misses their appointment. That's why this one matters so much. If you miss it, we will not be able to help you anymore. If you come to this appointment, you can join our Care Coach program. This program can help you with things like: Finding a place to live, or help with rent and deposits Food, meals, and help after a hospital stay Getting a personal care coordinator for your health Help if you've been in jail or prison recently Help with drugs, alcohol, or mental health A break if you're caring for a loved one Extra help if you're pregnant or just had a baby You can only book this appointment one time. There is no option to reschedule once you book, so pick a time you know you can make. If you book more than once, your extra booking will be canceled. We want to help you.
