If life feels impossible right now — if you're scared about housing, exhausted by mental health struggles, fighting to stay sober, or just trying to keep your family together — you are not invisible. You are seen. And you don't have to figure any of this out alone.
We are a peer-led nonprofit rooted in California's Central Valley, and we offer free support for adults who are dealing with some of life's hardest challenges. Not a hotline. Not a clipboard. Real people, real help, and a genuine belief that you can find a path forward.
Start here at our resources page — and let us help you figure out what's available for your situation.
Life in the Central Valley Is Hard. We Know.
The San Joaquin Valley — from Modesto and Stockton to Fresno and Merced — is one of the most economically challenged regions in California. Poverty rates here are among the highest in the state. Services are spread thin. Transportation is a constant obstacle. Heat makes survival harder. And too many systems require you to have it together before they'll help you get there.
We were built by and for people who know this reality from the inside. Our team includes people who have experienced homelessness, mental health crises, addiction, incarceration, and the specific exhaustion of trying to get help in a region that doesn't always make it easy.

What We Actually Do — And What Makes It Different
We don't hand you a pamphlet and send you home. We sit with you, figure out what's going on, and help you take the next step — whatever that step needs to be. Book a free appointment and let's start there.
Here's what's available:
Peer Support From People Who've Lived It
A peer support specialist is a real person who has been through it — housing instability, mental health struggles, substance use, trauma, incarceration — and found a way forward. They don't judge where you are. They walk with you, offer honest encouragement, and help you figure out your next step. This isn't therapy. It's someone genuinely in your corner.
Help With Housing When You Have Nowhere Stable to Land
Whether you're sleeping in your car in Stockton, couch-surfing in Fresno, facing an eviction notice in Modesto, or fleeing an unsafe situation — housing support starts with your specific situation. We help people navigate emergency options, transitional housing, rental assistance, landlord communication, and the paperwork that piles up when you're already overwhelmed.
Mental Health Support Without the Runaround
You shouldn't have to have a diagnosis before anyone talks to you. Mental health support here starts with a conversation — about what you're carrying, what's gotten heavy, and what you actually need. We help connect you to the right level of care for your situation, whether that's ongoing peer support, a referral to services, or just someone to help you process what you're going through.
If you need support right now, don't wait. Reach out to our team — free and no judgment. If you're in a mental health crisis, call 988 immediately.
Recovery and Substance Use Support, Without Judgment
Whether you're ready for treatment, exploring harm reduction, or just tired and unsure what you want — we meet you where you are. Your timeline matters. Your goals matter. Recovery support here is not a lecture or a set of requirements. It's a relationship with someone who gets it and wants to help you find what actually works for you.
Jobs, Income, and Getting Back on Your Feet
Gaps in your work history. A record. No childcare. Skills that don't show up well on a resume. These are real barriers, and none of them disqualify you from getting help with income and employment. We work with people on job readiness, employment navigation, and connecting with opportunities in the Central Valley that give people a real shot.
Medical Help, Even Without Insurance
Physical health, hygiene, and medical navigation are part of whole-person support. If you're on Medi-Cal — or think you might qualify — there may be far more covered services available to you than you've been told. We help people understand what they're entitled to and get actually connected to it.
Family, Relationships, and Finding Your Footing Again
Hard seasons put stress on every relationship. If family conflict, trust issues, or isolation are part of what you're carrying, support for your heart and relationships is available too. You don't have to compartmentalize your life to get help. We support the whole person.

Who We're Here For
We serve adults 18 and older across California's Central Valley — including Modesto, Stockton, Fresno, Merced, and the surrounding communities. You may find us especially helpful if you are:
Experiencing homelessness or housing instability — unsheltered, in a car, couch-surfing, in a shelter, facing eviction, or in an unsafe living situation
Struggling with mental health — depression, anxiety, PTSD, bipolar disorder, or anything that's making daily life hard to manage
Dealing with substance use — whether you're in recovery, thinking about it, or just trying to reduce harm
Recently released from incarceration and trying to rebuild
On Medi-Cal with complex needs you haven't been able to get help with
Pregnant or postpartum, especially if you're also facing housing or mental health challenges
Managing two or more chronic health conditions that affect your daily life
Feeling isolated, overwhelmed, or like you're carrying everything alone
If you're not sure whether you fit — that's exactly why we have our resources page. Fill out the free Total Resource Check-In and let us figure out what may be available for you.
The Barriers Are Real — Here's How We Work Through Them
You know what's in your way better than anyone. If you've been turned away, put on a list, or handed a pamphlet when you needed a person — we hear you. Here are the most common obstacles people face in the Central Valley, and how we work through them together:
Housing instability and eviction notices: When you're focused on where you're sleeping tonight, everything else feels impossible. We start with what's most urgent — emergency options, placement support, paperwork help — and we don't require you to have other parts of your life together first.
Stigma around mental health and substance use: Fear of judgment keeps a lot of people from reaching out. Our peer support team has been in those exact situations. There is no shame here. There is no wrong answer. The only requirement is showing up.
No transportation: Getting to appointments in Fresno, Merced, or rural parts of the Valley when you don't have a car is a real barrier. Ask about transportation support when you book your appointment. If you have Medi-Cal, free medical transportation may already be covered for you — we can help you find out.
No insurance or difficulty navigating healthcare: You don't need insurance to start. And if you have Medi-Cal, you may be entitled to basic needs and health support far beyond standard clinic visits. We help people understand their options in plain language — not policy language.
Employment gaps and history: A record, a gap, limited options — none of it disqualifies you. Employment and income support starts wherever you actually are right now. Small traction creates more traction.
Feeling too overwhelmed to even know where to start: That's what our Life Navigation support is for. Come in with everything tangled. We help you figure out which thread to pull first.
Could You Qualify for Free Extra Help Through Medi-Cal?
Most people with Medi-Cal in California don't know this: if your life is hard and complex, your Medi-Cal plan may be authorized to provide far more support than a standard doctor visit. This includes free, hands-on help with the real-life stuff that makes health outcomes hard — housing, transportation, food, care coordination, and more.
If you have Medi-Cal and you're dealing with any of the following, you may qualify: homelessness or housing instability, frequent ER visits, serious mental illness, substance use, recent incarceration, chronic health conditions, high-risk pregnancy, or disability.
Here's what qualifying members may be able to access through their existing Medi-Cal coverage:
A dedicated support person who coordinates all your care and stays in regular contact — not just when there's a crisis
Help scheduling and getting to appointments, including free transportation
Someone who talks to your doctors and providers on your behalf
Housing search, applications, and landlord communication support
Help keeping housing once you have it — paperwork, notices, problem-solving
Short-term housing if you've been discharged from a hospital and have nowhere safe to recover
Free delivered meals when medically needed
Connection to food support, legal navigation, and community services
This is not a cash payment. It's the real-life, hands-on coordination — covered through your existing Medi-Cal — that can mean thousands of dollars in actual support per year for qualifying members.
Your First Step: The Total Resource Check-In
Starting can feel like the hardest part. To find out if you qualify, fill out our free Total Resource Check-In. There are no wrong answers. The more honestly you answer, the more help we can potentially find for you. You're not committing to anything — you're just finding out what may be available.
Here is the simplest version of what to do next:
Go to helpishope.org/resources and fill out the Total Resource Check-In. It takes about 10 minutes on your phone. Answer honestly — there are no wrong answers.
You'll receive a personalized resource guide by email based on your specific situation, within 72 hours.
If you want one-on-one support, book a free appointment with a peer specialist who can walk you through next steps by phone or in person.
More boxes checked is a good thing. The form covers 16 areas of life. If a lot of them apply to you, that means more potential help — not less.
You don't have to call your Medi-Cal plan alone. We can help you prepare, make the call, and follow through.
You don't need to know the program names. You don't need to have a plan. Your job is to be honest about where you are. Ours is to turn that into action.
Frequently Asked Questions
Who do you serve?
We serve adults 18 and older in California's Central Valley — including Modesto, Stockton, Fresno, Merced, and surrounding communities — who are dealing with homelessness, mental health challenges, substance use, poverty, trauma, chronic illness, justice involvement, or overlapping life crises. If you're struggling, you belong here.
Is the support really free?
Yes. Our peer support, resource navigation, and Total Resource Check-In are all free. If you have Medi-Cal, additional services may be covered through your existing plan at no cost to you. Learn more on our resources page.
What if I don't know what kind of help I need?
That's completely okay. Book a Life Navigation appointment below and we'll figure it out together. You don't need to have it sorted before you reach out.
What if I've been let down by systems before?
We understand why trust is hard. Our team includes people who have been where you are — and found a way through. We don't operate from a clipboard. We operate from lived experience and a real commitment to showing up for you. One conversation is all we're asking for. Book it free here.
Do I need Medi-Cal to get help?
No. We can still connect with you and help you navigate your options even if you don't have Medi-Cal. If you think you might qualify, we can help you find out. Start with the Total Resource Check-In.
What if I have a criminal record?
A record does not disqualify you from support — it often makes you eligible for more. Reentry and reintegration support is part of what we do across the Central Valley.
I don't have a phone or email. Can I still get help?
A placeholder email can be used on the form. If you're having trouble accessing it at all, visit our website and reach out — we'll find a way to connect.
