If you have Medi-Cal through Health Net or Health Plan of San Joaquin, Housing Tenancy and Sustaining Services provide ongoing support to help you maintain stable housing. Help Is Hope care team connects eligible members to tenancy support specialists through partner providers who help with budgeting, landlord conflicts, and crisis intervention.

Medi-Cal covers Community Health Worker (CHW) services providing health education, navigation, advocacy, and resource connection through Health Net and Health Plan San Joaquin. CHWs speak your language, understand your culture, and work in your community as trusted health educators and navigators. Many members don't realize dentures are covered—a life-changing benefit for those with missing teeth. Poor oral health leads to serious health complications.
